WISH AGREEMENT AND GUIDELINES
FOR OUR SUPPORT, INFORMATION SHARING & INFORMATION STORAGE
Grant-A-Smile is a charity supporting parents and families battling chronic illness across London. We believe that your family would benefit from the valuable services and support that Grant-a-Smile can offer.
Your allocated Smile Hero/Volunteer will work with you to find out what household chores and services you would like us to carry out and how often we will visit. This information will form an action plan so you know exactly what you can expect from us and what we can expect from you.
This is a voluntary agreement and, as a registered social enterprise, there is no cost to you for our services. We will also endeavour to grant a special wish for your child or children in a bid to help create lasting memories and something positive to look forward to.
By signing this consent form you confirm that you agree to:
- Work with our Smile Heroes and Volunteers
- Allow Grant-A-Smile to talk to your child/children about their special wish
- Your information is shared with any other professional organisations who can offer support to you and your family.
For more information please visit our website: www.grantasmile.org.uk
Please complete and return the attached form so we can deliver our services as soon as possible. If you have any questions in the meantime, then please do not hesitate to get in contact.
