Wish Application form
We grant special wishes to children between the ages of 2-16 with the hope of putting a smile back on their faces and creating beautiful memories. If you are battling a chronic illness/es and your child/children deserve to have a special wish granted, please read, complete and submit each of the online forms at the bottom of this page. Alternatively, you can print copy and return the following PDF versions of the forms to admin@grantasmile.org.uk
